The fee for this service is currently $150.00 which must be rendered prior to the beginning of the evaluation and must be paid regardless of the outcome of the evaluation. If you are paying personally I regret that I cannot accept personal checks unless it is received well in advance of the evaluation and the check has had time to clear.
